I fully understand and acknowledge that the workshops offered at Meltdown Candle Bar may involve inherent risks, dangers and hazards which may result in injury or illness. I hereby voluntarily agree to release, waive, discharge, hold harmless, defend and indemnify Meltdown Candle Bar, its managers, members, and employees from any and all claims, actions or losses for bodily injury, property damage, wrongful death, or otherwise, which may arise from my participation in any workshops or classes, resulting products from workshops or classes, or from use of equipment provided by Meltdown Candle Bar. I accept FULL LIABILITY for creations made by participation in the workship and/or classes at Meltdown Candle Bar.
